    Summary

    Headaches are common in cocaine users, sometimes mimicking hemiplegic migraine or linked to withdrawal. Depression and brain lesions were also observed in this patient group.

    Background:

    • Cocaine use is prevalent among individuals admitted to municipal hospitals.
    • Headaches are a frequently reported symptom in this population.

    Observation:

    • A study reviewed 283 cocaine users, identifying 37 (13.1%) with headaches.
    • Patients were categorized into groups based on headache presentation and association with cocaine use or withdrawal.

    Findings:

    • Three patients experienced migraine-like headaches with transient hemiparesis, potentially linked to acute cocaine use.
    • Headaches associated with cocaine withdrawal were observed in five patients.
    • Among those without clear cocaine association, 22 had chronic daily headaches, nine with comorbid depression; three had focal brain lesions.

    Implications:

    • Headache is a significant complaint in hospitalized cocaine users.
    • Cocaine may precipitate a syndrome resembling hemiplegic migraine.
    • Depression, suicidal ideation, and structural brain disease are critical co-occurring conditions in cocaine users presenting with headaches.
