Long-term wear performance of reverse total shoulder arthroplasty (RTSA) is unknown. This study developed and tested a new RTSA wear simulator protocol, finding wear rates within the literature range, and suggesting multiple protocols for comprehensive testing.

Area of Science:

  • Orthopedic Surgery
  • Biomaterials Science
  • Biomechanical Engineering

Background:

  • Reverse total shoulder arthroplasty (RTSA) is a common surgical procedure for shoulder joint restoration.
  • Despite clinical acceptance, the long-term wear performance of RTSA implants remains largely uncharacterized.
  • Existing wear simulator testing protocols exhibit significant variability, hindering consistent evaluation.

Purpose of the Study:

  • To review current wear simulator testing methodologies for RTSA.
  • To develop a novel, standardized wear simulator protocol for RTSA.
  • To validate the developed protocol through a pilot study.

Main Methods:

  • A comprehensive literature review identified variations in existing RTSA wear simulation protocols.

  • Cadaveric testing and data from other research groups informed the determination of joint loading parameters.
  • An orbital-bearing simulator was modified with custom fixtures to replicate circumduction motion under adverse conditions.
  • Main Results:

    • The developed wear simulator protocol produced pilot study wear rates consistent with the broad range reported in the literature.
    • The protocol successfully simulated circumduction motion under mildly adverse conditions.
    • Significant variability in existing wear simulation protocols was confirmed.

    Conclusions:

    • The developed wear simulator protocol offers a standardized approach for evaluating RTSA implant wear.
    • A single protocol may be insufficient; a suite of diverse simulator protocols is recommended for comprehensive RTSA wear performance assessment.
    • Further research should focus on establishing a series of standardized protocols to fully characterize RTSA wear behavior.
