Background:

  • Primary dysmenorrhea (PDM) affects many young women, causing painful menstrual cramps.
  • Brain abnormalities are linked to PDM, but white matter (WM) changes are not well understood.

Purpose of the Study:

  • To investigate white matter (WM) microstructure alterations in women with primary dysmenorrhea (PDM).

Main Methods:

  • Diffusion tensor imaging (DTI) analyzed in 35 PDM patients and 35 healthy controls (HCs).
  • Tract-based spatial statistics and probabilistic tractography used to assess WM integrity.

Main Results:

  • PDM patients showed increased fractional anisotropy (FA) and decreased mean diffusivity (MD) and radial diffusivity (RD) in key WM tracts.
  • FA in specific brain regions correlated with PDM duration and severity.
  • Tractography revealed connections of these WM tracts to brain areas involved in pain and emotion processing.

Conclusions:

  • Preliminary evidence suggests white matter (WM) microstructure alterations in primary dysmenorrhea (PDM).
  • These findings offer insights into the underlying pathomechanism of PDM.
