Magnetic Resonance Imaging01:24

Magnetic Resonance Imaging

10.2K
Magnetic resonance imaging (MRI) is a noninvasive medical imaging technique based on a phenomenon of nuclear physics discovered in the 1930s, in which matter exposed to magnetic fields and radio waves was found to emit radio signals. In 1970, a physician and researcher named Raymond Damadian noticed that malignant (cancerous) tissue gave off different signals than normal body tissue. He applied for a patent for the first MRI scanning device in clinical use by the early 1980s. The early MRI...

Summary

Intramural hematoma (IMH), part of acute aortic syndrome, presents as aortic wall thickening. Understanding its imaging features and prognostic factors is crucial for timely treatment and improved outcomes in patients.

Area of Science:

  • Cardiovascular Imaging
  • Thoracic Radiology
  • Aortic Diseases

Background:

  • Intramural hematoma (IMH) is a component of acute aortic syndrome.
  • Historically, IMH was thought to arise without intimal tears, but intimomedial tears are now reported.
  • This blurs the distinction between IMH and aortic dissection, questioning acute aortic syndrome classifications.

Purpose of the Study:

  • Discuss the pathophysiology and differentiation controversies of IMH within acute aortic syndrome.
  • Review imaging prognostic factors for IMH.
  • Outline treatment strategies based on imaging findings and classification.

Main Methods:

  • Review of existing literature on IMH pathophysiology and imaging.
  • Analysis of imaging prognostic factors including Stanford classification, aortic diameter, IMH thickness, and effusions.
  • Discussion of medical versus surgical treatment strategies.

Main Results:

  • IMH natural history is variable, potentially leading to aneurysm, dissection, or rupture.
  • Key prognostic imaging factors include Stanford classification, aortic diameter, IMH thickness, focal contrast enhancement, and effusions.
  • Initial medical management shows success for Stanford type A IMH in some cohorts, with delayed surgery for complications.

Conclusions:

  • Accurate reporting of IMH imaging appearance and prognostic factors is vital.
  • Radiologists and surgeons must identify high-risk patients for appropriate treatment.
  • Improved understanding aids in optimizing patient outcomes for acute aortic syndromes.
