Summary

Early vitrectomy for acute post-cataract endophthalmitis can restore vision. This study shows 25-gauge vitrectomy effectively treated Staphylococcus epidermidis endophthalmitis, improving vision to 20/20.

Observation:

  • A 71-year-old male presented with symptoms 11 days post-cataract extraction, diagnosed with acute post-cataract endophthalmitis.
  • Initial visual acuity was counting fingers at 1 foot with significant ocular inflammation.

Findings:

  • Immediate 25-gauge vitrectomy with intravitreal antibiotics was performed.
  • Vitreous cultures and PCR identified Staphylococcus epidermidis.
  • Post-operatively, vision improved to 20/20 with resolution of inflammation.

Implications:

  • Minimally invasive vitrectomy is effective for acute post-cataract endophthalmitis.
  • Early intervention can lead to excellent visual recovery.
  • This approach offers a viable treatment option for severe endophthalmitis.
