Background:

  • Bacteroides fragilis is a common gut commensal.
  • Enterotoxigenic B. fragilis (ETBF) strains are linked to diarrhea and colorectal cancer (CRC).
  • Accurate detection of ETBF in stool is crucial for understanding its role in CRC.

Purpose of the Study:

  • To develop and evaluate a screening method for the Bacteroides fragilis toxin (bft) gene in human stool samples.
  • To compare ETBF carriage rates between CRC patients and age-matched healthy controls.

Main Methods:

  • Stool samples from 71 CRC patients and 71 controls were analyzed.
  • DNA was extracted using two methods: direct extraction and bacterial sweeps from agar cultures.
  • Standard and quantitative PCR (qPCR) were used to detect the bft gene.

Main Results:

  • The bft gene was detected more frequently using DNA from bacterial sweeps compared to direct DNA extraction.
  • Quantitative PCR (qPCR) demonstrated higher sensitivity than standard PCR.
  • ETBF carriage was identified in 27% of CRC patients versus 10% of controls (P=0.016).

Conclusions:

  • ETBF carriage is more prevalent in individuals with colorectal cancer.
  • Current molecular detection methods for ETBF in stool samples may have limited sensitivity.
  • Further optimization of DNA extraction and PCR techniques is needed for reliable ETBF detection.
