Negative Association of Diabetes With Thoracic Aortic Dissection and Aneurysm

Diabetes mellitus (DM) appears less common in patients with thoracic aortic dissection (TAD) or thoracic aortic aneurysm (TAA). This meta-analysis suggests a negative association between DM and these serious aortic conditions.

Purpose of the Study:
- To conduct a meta-analysis assessing the association between diabetes mellitus (DM) and the presence of thoracic aortic dissection (TAD) and/or thoracic aortic aneurysm (TAA).
Main Methods:
- A systematic literature search of MEDLINE and EMBASE databases was performed up to December 2015.
- Eleven eligible studies with a total of 47,827 participants were included in the meta-analysis.
- Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated to determine the association between DM and TAD/TAA.
Main Results:
- The primary pooled analysis of all 11 studies revealed a significantly lower prevalence of DM in individuals with TAD/TAA compared to controls (OR, 0.43; 95% CI, 0.31-0.59; P < .00001).
- Separate analyses indicated that both TAD (OR, 0.34; 95% CI, 0.19-0.61; P = .0003) and TAA (OR, 0.65; 95% CI, 0.45-0.94; P = .02) were significantly associated with a lower prevalence of DM.
- These findings were consistent across different analyses, highlighting a protective or inverse relationship.
Conclusions:
- Diabetes mellitus may be negatively associated with the presence of thoracic aortic dissection and/or thoracic aortic aneurysm.
- Further research is warranted to elucidate the underlying mechanisms of this observed inverse relationship.
- The findings suggest potential implications for risk stratification and management strategies for aortic diseases.
