Summary

Bilateral chronic subdural hematoma (CSDH) shares clinical similarities with unilateral CSDH but shows fewer midline shifts on CT scans. Most patients with CSDH, regardless of laterality, experience good surgical outcomes.

Background:

  • Chronic subdural hematoma (CSDH) is a frequent neurosurgical condition.
  • While often unilateral, CSDH can present bilaterally, with unclear clinical distinctions.

Purpose of the Study:

  • To investigate and compare the clinical characteristics of bilateral versus unilateral CSDH.
  • To identify potential differences in presentation, risk factors, and outcomes.

Main Methods:

  • Retrospective analysis of 120 surgically treated CSDH patients (2008-2012).
  • Patients categorized into bilateral and unilateral CSDH groups.
  • Comparison of clinical presentations, CT findings, complications, and outcomes.

Main Results:

  • Bilateral CSDH occurred in 10.9% of patients.
  • Lower, though not statistically significant, head injury rates in bilateral CSDH (36.4% vs. 59.6%).
  • Significantly less midline shift on CT scans in bilateral CSDH (p=0.010).

  • Bilateral CSDH exhibits similar clinical features and causes to unilateral CSDH.
  • A key difference is the reduced incidence of midline shift in bilateral cases.
