Urinary Incontinence Could Be Controlled by an Inflatable Penile Prosthesis

Background:

• Radical prostatectomy (RP) for prostate cancer frequently leads to postoperative urinary incontinence and erectile dysfunction (ED).
• Managing these dual complications presents a significant clinical challenge for patients and healthcare providers.

Purpose of the Study:

• To evaluate the efficacy of inflatable penile prosthesis (IPP) implantation in controlling urinary incontinence in patients experiencing both ED and incontinence post-RP.
• To assess patient satisfaction and functional outcomes following IPP implantation for dual-issue management.

Main Methods:

• A cohort of 25 patients with post-RP ED and incontinence underwent inflatable penile prosthesis implantation between March 2010 and May 2015.
• Urinary incontinence severity was assessed using the International Consultation on Incontinence Questionnaire-Short Form.
• Progressive inflation of the prosthesis was utilized over one month to manage urine leakage.

• Of 18 patients with available data, 13 (72.2%) achieved successful incontinence control with IPP.
• Mild to moderate stress incontinence was resolved in all 13 patients with partial prosthesis inflation (30%-60%).
• Three of five patients with severe total incontinence reported tolerating leakage with intermittent full inflation.

Conclusions:

• Inflatable penile prosthesis implantation is a highly recommended initial procedure for patients with concurrent ED and urinary incontinence post-radical prostatectomy.
• IPP offers a viable and effective solution for improving quality of life in this patient population.
