Prolonged computed tomography urography (CTU) effectively diagnoses duplex kidney anomalies. This extended imaging phase aids in visualizing duplicated ureters and affected kidney parenchyma, overcoming limitations of standard CTU.

Background:

  • Duplex kidney is a common congenital anomaly often leading to complications.
  • Standard four-phase computed tomography urography (CTU) can be insufficient for diagnosing duplex kidneys with prolonged dilatation.
  • Affected parenchyma and tortuous ureters in duplex kidneys may show delayed or absent opacification on typical CTU.

Purpose of the Study:

  • To evaluate the efficacy of prolonged-delay CTU in diagnosing duplex kidney anomalies.
  • To improve the visualization of duplicated ureters and affected renal parenchyma in duplex kidneys.

Main Methods:

  • Seventeen patients with suspected duplex kidney underwent prolonged CTU, including a delayed phase (1-72 hours).
  • Standard unenhanced and four-phase CTU were compared with the prolonged-delay CTU.
  • Diagnosis was confirmed using axial and 3D CT reconstructions from the prolonged CTU scans.

Main Results:

  • Unenhanced CTU showed only irregular perirenal morphology.
  • Standard four-phase CTU could not confirm duplex kidney diagnosis in affected patients.
  • Prolonged CTU, particularly the delayed phase, successfully opacified duplicated ureters and confirmed the diagnosis.
