Background:

  • Shigellosis is a significant public health concern, particularly in developing nations and among displaced populations.
  • A large influx of refugees into Greece in 2015 created conditions conducive to communicable disease outbreaks.

Observation:

  • A cluster of 16 shigellosis cases was identified in a refugee transit center in Athens between August and October 2015.
  • Both Shigella flexneri and Shigella sonnei were detected, with various serotypes identified.

Findings:

  • All tested isolates (n=13) exhibited multidrug resistance.
  • Seven isolates were identified as CTX-M-type extended-spectrum β-lactamase (ESBL) producers.
  • Pulsed-field gel electrophoresis suggested limited person-to-person transmission without secondary community cases.

Implications:

  • The detection of multidrug-resistant Shigella necessitates careful selection of empirical antibiotic treatments.
  • Real-time typing is crucial for effective epidemiological investigation and outbreak control in refugee settings.
  • Environmental hygiene and prompt treatment are vital for preventing shigellosis spread in displaced populations.
