Background:

  • Low back pain and sciatica can arise from pathologies distant from the spine.
  • Systemic lupus erythematosus (SLE) increases the risk of infection.

Observation:

  • A 59-year-old SLE patient presented with symptoms suggestive of lumbar spinal stenosis.
  • Initial treatment for septic arthritis of the wrist and spinal stenosis was ineffective.

Findings:

  • Further investigation revealed a large gluteal abscess as the underlying cause.
  • Drainage of the abscess and antibiotic therapy led to patient recovery.

Implications:

  • Unexplained or treatment-resistant low back pain in SLE patients warrants investigation for infections in atypical sites.
