Background:

  • Intermittent explosive disorder (IED) involves pathological impulsive aggression.
  • Previous research suggests frontal-limbic dysfunction and altered serotonin in IED.
  • White matter connections are crucial for social and emotional behavior, but IED's white matter integrity is understudied.

Purpose of the Study:

  • To investigate white matter connectivity using Diffusion Tensor Imaging (DTI) in individuals with IED.
  • To compare white matter integrity in IED patients, psychiatric controls, and healthy controls.

Main Methods:

  • 132 adults (18-55 years) with IED (n=42), psychiatric controls (n=50), and healthy controls (n=40) participated.
  • All participants were assessed while free of alcohol, drugs, and psychotropic medications.
  • Diffusion Tensor Imaging (DTI) and Tract-Based Spatial Statistics (TBSS) were used to analyze fractional anisotropy (FA) in white matter.

Main Results:

  • Individuals with IED showed significantly lower FA in two clusters within the superior longitudinal fasciculus (SLF) compared to both control groups.
  • Lower FA in these SLF clusters was associated with impulsive aggression and borderline personality disorder, but not psychopathy or antisocial personality disorder.
  • The findings indicate reduced white matter integrity in long-range frontal-temporoparietal connections in IED.

Conclusions:

  • IED is associated with compromised white matter integrity, specifically in the superior longitudinal fasciculus.
  • These white matter alterations may represent a neurobiological basis for the impulsive aggression seen in IED.
  • DTI provides valuable insights into the neural underpinnings of IED and related impulsive behaviors.
