Summary

Leukoencephalopathy with calcifications and cysts (LCC) is a rare neurological disorder. This study found reduced brain perfusion in affected white and gray matter in an LCC patient, suggesting potential links to cognitive decline.

Background:

  • Leukoencephalopathy with calcifications and cysts (LCC) is a rare, poorly understood neurological disorder.
  • Pathological hallmarks include microvascular changes, myelin loss, gliosis, and calcifications.

Observation:

  • A 55-year-old male patient with a 10-year history of LCC was studied.
  • Brain perfusion was assessed using pseudocontinuous arterial spin labeling (pCASL) MRI technique.

Findings:

  • The LCC patient exhibited diffuse hypoperfusion in white matter (WM), subcortical gray matter (GM), and cortical GM compared to healthy controls.
  • Hypoperfusion in WM and subcortical GM aligns with known LCC microangiopathy distribution.
  • Cortical hypoperfusion may be linked to cerebral atrophy or deafferentation, potentially contributing to severe impairment.

Implications:

  • This study highlights the utility of pCASL in evaluating LCC-related cerebral blood flow.
  • Findings suggest a potential role for hypoperfusion in the motor and cognitive deficits seen in LCC.
  • Further research into cerebral blood flow dynamics in LCC is warranted.
