Summary

Donepezil augmentation improved treatment-resistant obsessive-compulsive disorder (OCD) in aged patients. This add-on therapy was well-tolerated, showing significant symptom reduction in a small case series.

Background:

  • Obsessive-compulsive disorder (OCD) is a common and disabling psychiatric condition.
  • Many patients do not fully respond to standard serotonin selective reuptake inhibitor (SSRI) treatments.
  • Augmentation strategies are often necessary for treatment-resistant OCD.

Purpose of the Study:

  • To evaluate the efficacy and tolerability of donepezil as an add-on therapy for aged patients with treatment-resistant OCD.

Main Methods:

  • A retrospective case series of ten elderly patients with resistant OCD.
  • Patients received open-label donepezil (10 mg/d) as an augmentation to their regular pharmacotherapy.
  • Efficacy was measured using the Yale-Brown Obsessive Compulsive Scale and Clinical Global Impression scales after 8 weeks.

Main Results:

  • Donepezil augmentation was well-tolerated with no reported adverse events.
  • Significant reductions were observed in Yale-Brown Obsessive Compulsive Scale scores (27.3 to 16.9) and Clinical Global Impression-Severity scores (5.5 to 3.1).
  • Seven out of ten patients showed substantial clinical improvement.

Conclusions:

  • Donepezil appears to be a safe and potentially effective add-on treatment for elderly patients with treatment-resistant OCD.
  • Further investigation through randomized controlled trials is warranted to confirm these findings.
