Summary

The Swedish version of the Behavioral Pain Scale (BPS) is reliable for assessing pain in critically ill patients who cannot communicate. This validated tool aids in pain management for intubated and non-intubated individuals in intensive care units.

Area of Science:

  • Critical Care Medicine
  • Pain Assessment
  • Nursing Research

Background:

  • Assessing pain in critically ill patients is challenging due to limited communication from intubation and sedation.
  • International guidelines recommend behavioral pain assessment tools for non-communicative patients.
  • The Behavioral Pain Scale (BPS) is an internationally recognized instrument for this purpose.

Purpose of the Study:

  • To translate and adapt the Behavioral Pain Scale (BPS) into Swedish for use in intensive care units (ICUs).
  • To evaluate the inter-rater reliability and discriminant validity of the Swedish BPS version.
  • To assess the tool's applicability for both intubated and non-intubated critically ill patients.

Main Methods:

  • A ten-step translation and adaptation process was used to create the Swedish BPS.
  • The adapted Swedish BPS was tested on 20 critically ill patients (10 intubated, 10 non-intubated).
  • Inter-rater reliability and discriminant validity were assessed by comparing scores at rest and after repositioning.

Main Results:

  • The Swedish BPS demonstrated good inter-rater reliability with 85% agreement in the study sample.
  • The instrument showed discriminant validity, distinguishing pain levels between rest and post-procedure assessments.
  • The results support the utility of the Swedish BPS in clinical practice.

Conclusions:

  • The Swedish version of the BPS is a reliable and valid tool for assessing pain in critically ill patients unable to self-report.
  • Further research is recommended to explore the tool's reliability and validity further within the Swedish ICU context.
  • The validated Swedish BPS can improve pain management strategies for non-communicative ICU patients.
