Summary

Small-incision lenticule extraction (SMILE) surgery is safe and effective for correcting myopia in patients with posterior polymorphic corneal dystrophy (PPCD). SMILE successfully improved vision without worsening the underlying corneal condition.

Area of Science:

  • Ophthalmology
  • Refractive Surgery
  • Corneal Diseases

Background:

  • Posterior polymorphic corneal dystrophy (PPCD) is a rare, inherited corneal disorder.
  • Myopic refractive errors often require surgical correction.
  • The safety of refractive surgery in patients with PPCD is not well-established.

Observation:

  • A 37-year-old male with PPCD and myopia underwent small-incision lenticule extraction (SMILE).
  • Pre-operative assessment revealed reduced endothelial cell count in the affected eye.
  • The PPCD lesion was monitored post-surgery.

Findings:

  • Uncorrected distance visual acuity improved to 20/20 in both eyes 12 months post-SMILE.
  • The PPCD lesion showed no progression after the refractive surgery.

  • Endothelial cell count remained stable in the non-affected eye.
  • Implications:

    • SMILE may be a viable and safe surgical option for myopic patients with PPCD.
    • Further studies are warranted to confirm long-term safety and efficacy.
    • This case expands the understanding of refractive surgery options for patients with specific corneal dystrophies.
