Glomerular C3d deposits in ANCA-associated vasculitis indicate worse kidney survival and treatment response. This finding highlights C3d staining as a crucial prognostic factor for end-stage renal disease in patients with renal vasculitis.
Area of Science:
- Nephrology
- Immunology
- Pathology
Background:
- Pauci-immune necrotizing crescentic glomerulonephritis underlies ANCA-associated vasculitis.
- Complement activation is implicated in ANCA-associated vasculitis pathogenesis.
- Prognostic value of glomerular complement deposits is understudied.
Purpose of the Study:
- To evaluate the clinical and prognostic significance of C3d and C4d glomerular staining in renal vasculitis.
- To determine if C3d/C4d staining predicts end-stage renal disease (ESRD) in pauci-immune glomerulonephritis.
Main Methods:
- Retrospective analysis of 85 patients with pauci-immune necrotizing crescentic glomerulonephritis.
- Immunohistochemical staining for glomerular C3d and C4d deposits.
- Assessment of clinical parameters, treatment response, and renal survival (ESRD).
Main Results:
- C3d deposits were found in 49.4% of biopsies; C4d in 44.7%.
- C3d positivity correlated with renal impairment severity and lower treatment response (P=.003, P=.04).
- Renal survival was significantly lower in C3d-positive patients (51.8% at 5 years vs. 78.9% in C3d-negative).
- C3d staining independently predicted poor renal survival (HR 2.5, P=.03).
- C4d staining showed no significant impact on renal outcome.
Conclusions:
- Glomerular C3d deposition is an independent risk factor for ESRD in ANCA-associated renal vasculitis.
- C3d staining offers valuable prognostic information for kidney outcomes.
- C4d staining does not appear to be a significant prognostic marker in this cohort.
