The role of lymph node dissection (LND) in renal cell carcinoma (RCC) remains uncertain. Current evidence suggests LND is primarily for staging in advanced RCC, but more research is needed to clarify its impact.

To identify controversies, uncertainties, and future research directions regarding LND in RCC.

LND in RCC is mainly indicated for staging in cases with larger tumors, locally advanced disease, or adverse features like lymphadenopathy.
Salvage LND is being considered, but lacks definitive recommendations.
Limited and imperfect data hinder a clear assessment of LND's role and effect in RCC.

Conclusions:

Further basic, translational, and imaging research is crucial to elucidate the role of LND in RCC management.
The current understanding of LND's impact on RCC prognosis is limited by data quality.
