Background:

Remote monitoring programs offer innovative solutions for heart failure disease management.
Understanding the components and impact of these programs is vital for healthcare agencies.

Purpose of the Study:

To examine a remote monitoring heart failure disease management program using the Donabedian Quality and Logic Model.
To identify factors influencing program effectiveness and resource allocation.

Main Methods:

Quantitative, post-test only case-matched design.
Involved 210 participants across 23 home health care agencies.

Main Results:

An inverse relationship was found between caregiver support and decision-making complexity (X²(3, n=210)=29.984, P=0.012).
Participants advised to go to the ER had a 1.00 probability of doing so, versus 0.37 if advised otherwise.
A 0.50 probability existed for patient education or clinical intervention prior to hospital readmission.

Conclusions:

Caregiver support is a critical structural component in remote monitoring programs for heart failure.
Nurses' decision-making, influenced by caregiver support, may mitigate hospital utilization.
Home health agencies should consider family and social support when implementing remote monitoring systems.
