Integrated Healthcare System01:20

Integrated Healthcare System

2.5K
An integrated healthcare system (IHS) is a set of organizations that provides for or arranges to provide coordinated and continuous service to a defined population. The IHS takes responsibility for that particular population's health status and outcome, both clinically and fiscally. An integrated healthcare system is a well-organized, well-coordinated, and collaborative network. The integrated delivery system is a network that connects different healthcare providers to deliver organized,...

Issues And Trends In Healthcare Delivery System

6.4K
The issues and trends in healthcare delivery are constantly changing. The COVID-19 pandemic is one recent issue that wreaked havoc on healthcare systems, causing a shortage of healthcare workers, high demand for medicines and supplies, and increased medical expenditure due to a lack of insurance. Other issues include rising healthcare costs and care fragmentation.

Area of Science:

  • Biomedical Informatics
  • Health Data Interoperability

Background:

  • Informatics for Integrating Biology and the Bedside (i2b2) repositories store valuable patient data.
  • Accessing and integrating this data for research and clinical applications can be challenging.
  • Standardized data formats are crucial for interoperability.

Purpose of the Study:

  • To develop an interface for serving i2b2 patient data in Fast Healthcare Interoperability Resources (FHIR) format.
  • To enable secure, per-patient data access for third-party applications using the SMART on FHIR specification.
  • To transform i2b2 into a platform for innovative healthcare applications.

Main Methods:

  • Developed a SMART-on-FHIR cell as an i2b2 server plug-in.
  • Implemented modules for authentication, REST API, i2b2-to-FHIR conversion, resource enrichment, query engine, and caching.
  • Utilized the substitutable, modular, third-party applications (SMART) OAuth2 specification for authorization.
  • Tested the cell with a test i2b2 installation and a SMART application.

Main Results:

  • Successfully served patient data from i2b2 in FHIR format.
  • Demonstrated secure access to demographics, medications, laboratory results, and diagnoses for test patients.
  • Confirmed that a SMART application could launch and access i2b2 data via the cell.
  • Made the source code freely available as open source.

Conclusions:

  • The SMART-on-FHIR cell simplifies and secures data access for i2b2 sites.
  • It promotes innovation and interoperability by leveraging FHIR standards.
  • The solution effectively transforms i2b2 into a versatile application platform.
