    Context:

    • Xanthogranulomatous cholecystitis (XGC) is a rare gallbladder inflammation with a distinct histological appearance.
    • Clinical characteristics, prognostic, and physiopathologic implications of XGC remain largely unknown.

    Purpose:

    • To analyze and compare clinical characteristics, risk factors, morphologic expression, complications, and mortality between patients with XGC and non-XGC gallbladder inflammation.
    • To determine if XGC represents a distinct clinical entity.

    Summary:

    • A study of 40 XGC patients and 80 non-XGC patients revealed XGC is more frequent in males (p < .05) and occurs in 1.8% of cholecystectomies.
    • While XGC showed a higher frequency of acute pyogenic inflammation and biliary complications like duct compression and fistulization, there was no significant difference in overall complication rates or mortality compared to non-XGC cases.
