Background:

  • Primary liver carcinosarcoma is an extremely rare malignancy characterized by coexisting sarcomatous and carcinomatous components.
  • Hepatocellular carcinoma and rhabdomyosarcoma are the typical histological subtypes observed in liver carcinosarcoma.

Observation:

  • Initial staging 18F-fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT) revealed intense FDG uptake in a patient diagnosed with liver carcinosarcoma.
  • The patient's tumor was histologically confirmed to be a composite of hepatocellular carcinoma and rhabdomyosarcoma.

Findings:

  • Two months post-surgical resection, a restaging PET/CT demonstrated rapid disease progression.
  • Metastases were identified in the abdomen and lungs, highlighting the aggressive nature of this rare liver cancer.

Implications:

  • This case underscores the critical importance of early restaging 18F-fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT) after surgical resection.
  • Timely monitoring is essential for managing patients with liver carcinosarcoma due to its aggressive behavior and potential for rapid progression.
