The corrections department: the forgotten branch of the mental health system
Insights
Hawaii
Area of Science:
- Criminology
- Public Health
- Psychiatry
Background:
- The deinstitutionalization of mental health patients in the 1960s and 1970s led to an increase in mentally ill individuals within correctional facilities.
- Correctional facilities, particularly jails, became de facto mental health institutions due to inadequate community services.
- This shift has created significant challenges for corrections systems unprepared to manage seriously mentally ill inmates.
Purpose of the Study:
- To examine the challenges faced by the Hawaii Department of Corrections in managing incarcerated mentally ill individuals.
- To highlight the historical context of deinstitutionalization and its impact on correctional populations.
- To underscore the unpreparedness of correctional facilities in addressing the needs of seriously mentally ill inmates.
Main Methods:
- Qualitative analysis of the historical context and systemic issues.
- Review of the impact of deinstitutionalization policies on correctional populations.
- Examination of the role of correctional facilities as unintended mental health service providers.
Main Results:
- Correctional facilities, including jails, are ill-equipped to manage the complex needs of seriously mentally ill inmates.
- The lack of community-based mental health services has shifted the burden onto the corrections system.
- Mentally ill individuals are often incarcerated without adequate treatment, exacerbating their conditions and posing challenges for facility staff.
Conclusions:
- The incarceration of mentally ill individuals highlights a systemic failure in providing adequate community-based mental health care.
- Correctional facilities are ill-suited to serve as long-term mental health treatment centers.
- Urgent policy changes are needed to address the mental health crisis within the criminal justice system.
Abstract:
The Hawaii Department of Corrections (DOC), like other corrections systems throughout the nation, struggles with problems arising from incarcerating mentally ill prisoners and detainees. The incarcerated population of mentally ill persons burgeoned as mental hospital inmates were "deinstitutionalized" during the 1960s and 1970s. However, correctional facilities, particularly jails, became alternative institutions for mentally ill persons when communities were unprepared to provide required services. Correctional facilities were equally unprepared to manage seriously disabled mentally ill (SDMI) inmates, but the facilities provided a dumping ground for a community's undesirables. Out of sight, out of mind!
