Summary

The bispectral index (BIS) and intraoperative electrocorticography (iECoG) show a nonlinear correlation during epilepsy surgery. Burst suppression patterns were absent with BIS scores above 60, indicating BIS is a variable measure.

Background:

  • Intraoperative electrocorticography (iECoG) guides epilepsy surgery.
  • Bispectral index (BIS) monitors anesthesia depth, including in epilepsy surgery.
  • Correlation between BIS and iECoG during epilepsy surgery is not well understood.

Purpose of the Study:

  • To evaluate the correlation between BIS values and iECoG parameters.
  • To optimize iECoG recording quality and duration in epilepsy surgery.

Main Methods:

  • Prospective study of 28 epilepsy surgery patients (Sept 2008-Oct 2013).
  • Anesthesia with propofol and fentanyl.
  • Correlated BIS values with iECoG parameters: burst suppression (BS), suppression time, background frequency, and Mathern et al. iECoG score.

Main Results:

  • Nonlinear polynomial cubic relationship observed between BIS and iECoG parameters.
  • BIS 40-60: iECoG BS <5s, background frequency 10-17 Hz, iECoG score 2.
  • No iECoG burst suppression observed with BIS > 60.

Conclusions:

  • BIS and iECoG parameters exhibit a nonlinear correlation during propofol-fentanyl anesthesia in epilepsy surgery.
  • BIS > 60 reliably indicates the absence of burst suppression patterns.
  • BIS is a nonlinear, multidimensional measure with high variability concerning iECoG parameters.
