Aneurysm II: Clinical Manifestations and Diagnostic Studies01:21

Aneurysm II: Clinical Manifestations and Diagnostic Studies

486
Thoracic, aortic arch and abdominal aneurysms are significant vascular conditions that can present with various clinical manifestations and lead to serious complications. Understanding these manifestations and the appropriate diagnostic studies is essential for effective management and treatment.Thoracic Aortic AneurysmsThoracic aortic aneurysms often remain asymptomatic until they reach a size that impinges on adjacent structures. They typically cause deep, diffuse chest pain that radiates to...

Appendicitis-I: Introduction01:22

Appendicitis-I: Introduction

3.3K
The appendix, a small, narrow, blind tube extending from the inferior part of the cecum, is widely regarded as a vestigial organ, having lost much of its original function through evolution. Despite its diminished role, the appendix can become inflamed, a condition known as appendicitis.
Etiology: Appendicitis can arise from various causes, primarily rooted in the obstruction of the appendix lumen. Factors contributing to this obstruction include fecal accumulation, lymphoid hyperplasia and, in...

Esophageal Varices-I: Introduction01:24

Esophageal Varices-I: Introduction

2.0K
Esophageal varices are dilated, tortuous veins which are found mainly in the submucosa of the lower esophagus but which may also appear higher up or extend into the stomach. They develop due to increased pressure in the portal venous system, often as a result of liver cirrhosis. This condition scars and damages the liver, impeding normal blood flow through the portal vein. To compensate, blood seeks alternative pathways, forming fragile new vessels (varices) in the esophagus and stomach. These...

Venous Thrombosis II: Clinical Manifestations and Diagnostic Studies

422
The key difference between Superficial Vein Thrombosis (SVT) and Deep Vein Thrombosis (DVT) lies in their location and severity.Clinical ManifestationsSVT typically presents with localized pain, tenderness, and redness along the course of a superficial vein, often accompanied by a palpable, cord-like structure under the skin. This condition is usually less dangerous than DVT but can be uncomfortable and may lead to complications such as cellulitis or, rarely, a clot extension into the deep...

Inflammatory Bowel Disease I: Ulcerative Colitis

1.4K
Introduction
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...

Inflammatory Bowel Disease II: Crohn's Disease

1.5K
Introduction
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
Crohn's disease
Crohn's disease is a chronic, systemic inflammatory bowel disease (IBD) that predominantly affects the gastrointestinal tract. It is marked by...

    Summary

A 43-year-old man with metastatic lung cancer experienced acute adrenal insufficiency due to bilateral adrenal metastases. Prompt hormone replacement and fluid resuscitation saved his life, highlighting a rare presentation of adrenal crisis.

    Area of Science:

    • Endocrinology
    • Oncology
    • Radiology

    Background:

    • Acute adrenal insufficiency is a life-threatening endocrine emergency.
    • Metastatic carcinomatosis typically involves organs other than the adrenal glands.

    Observation:

    • A 43-year-old male presented with abdominal distension, confusion, and vascular collapse.
    • Radiologic imaging revealed massive bilateral adrenal enlargement.
    • Initial differential diagnoses included sepsis or hemorrhage.

    Findings:

    • Laboratory tests rapidly confirmed acute Addisonian crisis.
    • The patient had a history of metastatic lung cancer.
    • The adrenal enlargement was attributed to bilateral adrenal metastases from lung cancer.

    Implications:

    • This case highlights an uncommon presentation of adrenal insufficiency secondary to metastatic adrenal disease.
    • Early recognition and treatment with adrenocorticoid therapy and fluids are crucial for survival.
    • Distinguishing adrenal metastases from other causes of adrenal enlargement is vital in oncology patients.
