Increasing demand for total joint arthroplasty necessitates sustainable care models.
Medicare is the primary payer for a majority of hip and knee arthroplasty episodes.
Projected 673% increase in total hip and knee arthroplasty by 2030 highlights the need for efficient care.

Purpose of the Study:

To compare orthopedic cohort groups with and without defined postacute care pathways.
To evaluate the effects of postacute care pathways on service utilization and cost for Medicare patients.
To analyze outcomes within the Bundled Payments for Care Improvement (BPCI) program.

Main Methods:

Analysis of claims data for elective hip and knee arthroplasty episodes from a national BPCI database.
Independent reviewers identified groups with defined clinical pathways.
Comparison of outcomes including postacute care costs, utilization rates (IRF, SNF, home health), and readmissions.

Main Results:

Orthopedic physicians with defined postacute care pathways showed significant cost reductions: $3189 for hip arthroplasty and $2466 for knee arthroplasty per episode.
