    Summary

This study introduces a new functional factor of liver parenchyma (FA-hepatogram) derived from 99mTc-PMT hepatobiliary scintigraphy. This method effectively evaluates global liver function, outperforming conventional methods in complex cases.

    Area of Science:

    • Nuclear Medicine
    • Hepatology
    • Medical Imaging

    Background:

    • Assessing global liver function is crucial for diagnosing and managing liver diseases.
    • Conventional hepatogram methods using Region of Interest (ROI) analysis have limitations in complex liver conditions.
    • There is a need for a more robust method to evaluate overall liver function.

    Purpose of the Study:

    • To develop and validate a novel functional factor of liver parenchyma (FA-hepatogram) using factor analysis.
    • To compare the efficacy of the FA-hepatogram with conventional methods for liver function evaluation.
    • To assess the correlation of FA-hepatogram parameters with established liver function tests.

    Main Methods:

    • Factor analysis was applied to hepatobiliary scintigraphy data obtained with 99mTc-N-pyridoxyl-5-methyltryptophan (99mTc-PMT).

    Related Experiment Videos

  • Two key parameters, peak time and T 1/2, were derived from the FA-hepatogram.
  • Correlation with Indocyanine Green Retention Rate at 15 minutes (ICG-R15) was assessed.
  • Main Results:

    • The FA-hepatogram demonstrated a good correlation with ICG-R15.
    • Parameters derived from FA-hepatogram showed a progressive increase in normal volunteers, chronic hepatitis, compensated cirrhosis, and decompensated cirrhosis.
    • FA-hepatogram proved effective in evaluating global liver function even in cases with intrahepatic masses, intrahepatic gall bladder, biliary dilatation, or severe cirrhosis, where ROI-hepatograms were unsuitable.

    Conclusions:

    • The FA-hepatogram derived from 99mTc-PMT scintigraphy is a valuable tool for assessing global liver function.
    • This novel method offers superior performance compared to conventional ROI-based hepatograms, especially in challenging clinical scenarios.
    • FA-hepatogram parameters provide a sensitive measure of liver parenchymal function across a spectrum of liver conditions.
