Background:

Scrotal leiomyosarcoma is an exceptionally rare malignancy originating from the dartos muscle layer.
Fewer than 40 cases have been documented globally, highlighting its rarity.

Observation:

A 48-year-old male presented with a firm, irregular, non-tender scrotal mass.
Initial presentation suggested a rare scrotal tumor.

Findings:

The patient was diagnosed with scrotal leiomyosarcoma.
Wide surgical excision with clear margins was performed.
A 14-month follow-up revealed no evidence of tumor recurrence.

Implications:

This case reinforces wide surgical excision as an effective treatment for scrotal leiomyosarcoma.
Early diagnosis and complete resection are crucial for favorable outcomes in these rare tumors.
