Summary

Digital glissonectomy, a safe procedure for superficial Glisson's capsule deposits, is a feasible option in cytoreductive surgery for peritoneal carcinomatosis. This technique does not increase morbidity or mortality associated with the overall procedure.

Background:

  • Complete cytoreductive surgery (CRS) is the only curative treatment for peritoneal carcinomatosis.
  • Superficial Glisson's capsule involvement historically managed with electroevaporation or resection.
  • Digital glissonectomy offers a standardized approach for these cases.

Purpose of the Study:

  • To present the standardized digital glissonectomy technique.
  • To evaluate the safety and feasibility of digital glissonectomy.

Main Methods:

  • Standardized digital glissonectomy procedure implemented since 2009.
  • Retrospective analysis of 91 patients undergoing glissonectomy during CRS (2009-2014).

Main Results:

  • Glissonectomy performed in 14% of 655 CRS patients, primarily for pseudomyxoma peritonei and high peritoneal cancer index.
  • No significant increase in morbidity (p=0.069) or mortality (p=0.949) observed after glissonectomy.
  • Digital glissonectomy was frequently performed in patients with high peritoneal cancer index.

Conclusions:

  • Digital glissonectomy is a feasible and safe surgical option.
  • Indicated for superficial peritoneal carcinomatosis deposits on the Glisson's capsule.
  • Does not adversely affect overall CRS outcomes.
