Summary

The harmonic scalpel FOCUS is safe for the recurrent laryngeal nerve (RLN) during thyroid surgery. Studies show no histological damage to the RLN, even when the device is used as close as 1 mm away.

Background:

  • The harmonic scalpel is frequently used in thyroidectomies.
  • Concerns exist regarding its safety near the recurrent laryngeal nerve (RLN).

Purpose of the Study:

  • To evaluate the safety of the harmonic scalpel FOCUS during thyroidectomy procedures.
  • To determine the risk of thermal injury to the recurrent laryngeal nerve (RLN).

Main Methods:

  • Ten pigs underwent dissection of 20 recurrent laryngeal nerves (RLNs) using the harmonic scalpel FOCUS.
  • Nerve-to-instrument distances were measured.
  • Histological examination of RLNs was performed after euthanasia.

Main Results:

  • No immediate thermal damage to the RLN was observed, even at a distance of 1 mm.
  • Histological analysis confirmed no nerve lesions after device application near the RLN.

Conclusions:

  • The harmonic scalpel FOCUS is safe for surrounding neural structures during thyroid surgery.
  • Close application (1 mm) of the device to the RLN did not result in histological damage.
