An 82-year-old man with Addison disease and osteoporosis experienced elevated uptake in a bladder stone detected via bone scintigraphy and PET/CT imaging.

- Further investigation may be warranted to understand the mechanism of F-NaF and Tc-MDP uptake in bladder stones.
- Consideration of bladder stones in the differential diagnosis for abnormal pelvic uptake in bone imaging studies is suggested.
