Summary

Chronic fatigue syndrome (CFS) remains poorly understood, with no definitive cause identified. Current treatments like cognitive behavior therapy and graded exercise therapy offer moderate effectiveness for managing CFS symptoms.

Background:

Chronic fatigue syndrome (CFS) significantly impacts patients and society, causing productivity losses, informal costs, and medical expenses.
High prevalence (0.2-6.4%) and low employment rates (27-41%) highlight the societal burden of CFS.
Loss of productivity is the most substantial economic cost associated with CFS.

Purpose of the Study:

To systematically review recent scientific literature on chronic fatigue syndrome/myalgic encephalomyelitis (CFS/ME).

Main Methods:

Literature search of MEDLINE, Cochrane Library, and Web of Sciences databases.
Inclusion of articles published between January 1, 1990, and April 1, 2015.
Screening of reference lists from selected articles for additional relevant studies.

Main Results:

No proposed etiological factors for CFS have demonstrated strong, reproducible scientific evidence.
The biopsychosocial model is increasingly favored over the biomedical model for understanding CFS.
The exact etiological mechanism underlying CFS remains unknown.

Conclusions:

Due to the unknown etiology, disease-specific treatments for CFS are currently unavailable.
Cognitive behavior therapy (CBT) and graded exercise therapy (GET) have shown moderate effectiveness in managing CFS.
Further research is needed to elucidate the underlying mechanisms and develop targeted treatments for CFS.
