Summary

Two methods for preparing Bowman layer (BL) grafts demonstrated similar success rates for BL transplantation. Both whole donor globes and corneoscleral rims are viable sources for BL grafts.

Background:

  • Bowman layer (BL) transplantation is a promising surgical technique for treating various corneal pathologies.
  • Standardized and efficient preparation methods for BL grafts are crucial for successful transplantation outcomes.

Purpose of the Study:

  • To describe and compare two distinct preparation techniques for Bowman layer (BL) grafts intended for BL transplantation.
  • To evaluate the feasibility and success rates of these two BL graft preparation methods.

Main Methods:

  • A retrospective analysis compared two techniques for preparing BL grafts: Technique I from whole donor globes and Technique II from previously excised corneoscleral buttons.
  • Data on graft preparation difficulties and failure rates were collected and analyzed for both techniques.

Main Results:

  • Overall success rate for BL graft preparation was 70.8% (51/72).
  • Technique I yielded a 69.4% success rate (25/36), while Technique II achieved a 72.2% success rate (26/36).
  • The primary reasons for graft discard were tearing of the BL tissue (19 cases) and attached stroma (2 cases).

Conclusions:

  • Isolated Bowman layer (BL) grafts can be successfully prepared from both whole donor globes and corneoscleral rims with comparable success rates.
  • Utilizing corneoscleral rims offers a potential advantage, allowing for simultaneous preparation of grafts for Descemet membrane endothelial keratoplasty (DMET) from the posterior layers and BL grafts from the anterior portion of the same donor cornea.
