Background:

Investigating purine metabolism in individuals with low plasma urate concentrations.
Understanding the metabolic response to physical exertion in humans.

Observation:

A 37-year-old healthy male subject with very low plasma urate was studied.
High-performance liquid chromatography (HPLC) was used to measure purine concentrations in plasma and urine.
Persistent elevation of xanthine and a slight increase in hypoxanthine were observed.

Findings:

Exercise, including intensive treadmill and long-distance running, was investigated.
Hypoxanthine levels increased to control levels post-exercise but showed slower plasma elimination.
Xanthine levels remained largely unchanged despite physical exertion.
