  • Kawasaki disease (KD) diagnosis in infants is challenging, often presenting only with fever.

Observation:

  • A 6-month-old infant presented with prolonged fever and bilateral pleural effusion.
  • Initial workup showed elevated inflammatory markers, thrombocytosis, and hypoalbuminemia, with a normal abdominal ultrasound.
  • The infant was initially suspected of having incomplete Kawasaki disease.

Findings:

  • The infant was ultimately diagnosed with perforated retrocecal appendicitis and abscess formation.
  • The non-specific presentation and normal initial imaging complicated the diagnosis.

Implications:

  • Physicians must consider appendicitis in infants with non-specific symptoms, even when other diagnoses are suspected.
  • Timely diagnosis and treatment of infantile appendicitis are vital to reduce morbidity and mortality.
  • This case highlights the diagnostic challenges of appendicitis in young children.
