The lateral view of the cranium is dominated by temporal, sphenoid, and ethmoid bones.

Background:

Pilonidal sinus is a chronic inflammatory condition typically affecting the sacrococrocgeal region.
It is characterized by sinus tracts and often associated with hair follicles.

Observation:

A rare case of pilonidal sinus located in the lateral orbital region is presented.
The patient was initially misdiagnosed with an epidermal cyst.

Findings:

Incomplete excision of the sinus tract resulted in significant morbidity.
The condition led to the destruction of bone tissue in the lateral orbital wall.

Implications:

Accurate and timely diagnosis of orbital pilonidal sinus is crucial.
Prompt treatment is necessary to prevent progression to osteomyelitis and further bone destruction.
