The Availability Heuristic

A heuristic is a general problem-solving framework (Tversky & Kahneman, 1974). You can think of these as mental shortcuts that are used to solve problems. Different types of heuristics are used in different types of situations, and the impulse to use a heuristic occurs when one of five conditions is met (Pratkanis, 1989):

Summary

Topiramate, an anticonvulsant, is used for headaches, neuropathic pain, and bipolar disorders. Therapeutic drug monitoring (TDM) for topiramate is considered possibly useful due to dose-dependent effects and concentration variability.

Context:

Topiramate, a second-generation anticonvulsant, has been available in France since 1997.
It is indicated for headache prophylaxis and used off-label for neuropathic pain and bipolar disorders.

Purpose:

To evaluate the utility of therapeutic drug monitoring (TDM) for topiramate.
To assess the relationship between topiramate dosage, plasma concentrations, and clinical outcomes.

Summary:

Topiramate's efficacy and adverse reactions are dose-dependent.
While plasma concentrations correlate well with dosage and show low interindividual variability, there's an overlap between effective, ineffective, and toxic concentration ranges.
No validated therapeutic range exists, though typical concentrations are 5-20 mg/L.
