Calcaneus osteomyelitis secondary to Guthrie test: case report

Neonatal calcaneus osteomyelitis, a rare heel bone infection, can arise from invasive procedures like the Guthrie test. Early diagnosis and treatment led to recovery without surgery in this case.

Background:
- Calcaneus osteomyelitis is an uncommon infection in newborns, often linked to invasive procedures, prematurity, and low birth weight.
- Clinical signs can be subtle, making diagnosis challenging.
Observation:
- A neonate presented with heel swelling, redness, and increased temperature following a Guthrie test.
- Ultrasound revealed a fluid collection, and Staphylococcus aureus was isolated from drained fluid.
- MRI confirmed inflammation and tissue defects in the foot.
Findings:
- The neonate was treated with intravenous and oral antibiotics for six weeks.
- Clinical symptoms resolved completely without the need for surgical curettage.
Implications:
- Calcaneus osteomyelitis should be considered in neonates with heel swelling after invasive procedures.
- This case highlights the importance of recognizing subtle signs and prompt management of neonatal bone infections.
