Area of Science:

  • Spinal Surgery
  • Orthopedics
  • Minimally Invasive Procedures

Background:

  • Minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) is increasingly used for degenerative disc disease.
  • Traditional pedicle screw insertion techniques are being used less frequently.
  • The accuracy of novel pedicle screw insertion methods requires evaluation.

Purpose of the Study:

  • To analyze the accuracy of the true anteroposterior view pedicle screw insertion technique in MIS-TLIF.
  • To compare this technique with conventional pedicle screw insertion.
  • To discuss the clinical application value of the true anteroposterior view technique.

Main Methods:

  • A comparative study involving 52 patients (group A) using the true anteroposterior view and 87 patients (group B) using conventional pedicle screw insertion.
  • Patients were diagnosed with lumbar disc herniation or spinal stenosis.
  • Key metrics included screw placement time, irradiation exposure, accuracy, and neurovascular injury incidence.

Main Results:

  • Group A demonstrated significantly reduced screw placement time and intraoperative irradiation exposure.
  • Pedicle wall penetration rates were significantly lower in group A (1.44% lateral, 0% medial, 2.40% anterior).
  • Group A had no serious complications, while group B had two cases requiring revision due to medial penetration.

Conclusions:

  • The true anteroposterior view pedicle screw insertion technique is safe, accurate, and efficient for MIS-TLIF.
  • This technique offers reduced operative time and radiation exposure.
  • It achieves a higher accuracy rate in pedicle screw placement.
