Insights

Asparagine synthetase deficiency is a rare neurometabolic disorder causing severe developmental delay and brain malformations. This study identifies new mutations in the ASNS gene, expanding the known genetic causes and highlighting consistent clinical features for easier diagnosis.

Area of Science:

  • Genetics
  • Neurology
  • Metabolic Disorders

Background:

  • Asparagine synthetase deficiency (OMIM# 615574) is a rare neurometabolic disorder.
  • Characterized by congenital microcephaly, severe global developmental delay, intractable seizures, or hyperekplexia.
  • Brain MRI shows cerebral atrophy, simplified gyral pattern, and delayed myelination.

Observation:

  • Two families with affected members were studied.
  • Patients presented with microcephaly, severe hyperekplexia, and brain malformations including simplified gyral pattern and hypoplastic cerebellum/pons.
  • Whole exome sequencing identified novel homozygous mutations in the ASNS gene.

Findings:

  • Identified two novel homozygous mutations in the ASNS gene: c.970C > T:p.(Arg324*) and c.944A > G:p.(Tyr315Cys).
  • Expanded the known mutational spectrum for asparagine synthetase deficiency.
  • Observed remarkable clinical homogeneity among affected individuals.

Implications:

  • Facilitates recognition and molecular confirmation of asparagine synthetase deficiency.
  • Aids in timely genetic counseling for affected families.
  • Contributes to understanding the genetic basis of severe neurodevelopmental disorders.
