Background:

  • Acute scrotum is a medical emergency, with testicular torsion accounting for approximately 25% of cases.
  • The incidence of testicular torsion is around 1 in 4,000 individuals under 25, peaking in adolescents aged 12-18.
  • While often idiopathic, testicular torsion can be linked to anatomical, traumatic, or environmental factors.

Observation:

  • A 15-year-old male presented with sudden, continuous left testicular pain radiating to the abdomen and groin, accompanied by nausea and vomiting.
  • Symptoms had been present for over 12 hours.
  • Doppler ultrasound revealed findings consistent with testicular torsion.

Findings:

  • Surgical exploration confirmed a 360° rotation of the spermatic cord.
  • The left testicle was necrotic.
  • A left orchiectomy was performed due to the irreversible damage.

Implications:

  • Testicular torsion must be a primary consideration in cases of acute scrotal pain.
  • Timely diagnosis is critical and directly impacts the likelihood of testicular salvage.
  • Delay in diagnosis significantly increases the risk of testicular loss, necessitating urgent surgical intervention.
