Area of Science:

  • Oral and Maxillofacial Surgery
  • Orthodontics
  • 3D Imaging in Medicine

Background:

  • Conventional bimaxillary orthognathic surgery planning is resource-intensive and prone to errors.
  • Three-dimensional (3D) virtual planning offers a more efficient and reliable alternative.
  • Skeletal III malocclusion with facial asymmetry presents complex surgical challenges.

Observation:

  • A patient with Class III incisor relationship, Skeletal III base, facial asymmetry, reverse overjet, and anterior open bite was treated.
  • Combined cone beam computed tomography (CBCT) and dental scan data were used for 3D planning.
  • A new technique for optimizing intercuspation by scanning occluded dental casts was implemented.

Findings:

  • 3D virtual planning demonstrated high accuracy and improved treatment outcomes.
  • The 3D planning approach significantly reduced laboratory time compared to conventional methods.
  • Integration of final dental occlusion into 3D planning facilitated optimal intercuspation.

Implications:

  • 3D virtual planning enhances precision and efficiency in complex orthognathic surgeries.
  • This technology offers a cost-effective and reliable alternative for surgical planning.
  • The described method for incorporating occlusal data improves functional and aesthetic results in orthognathic surgery.
