Area of Science:

  • Nuclear Medicine
  • Oncology Imaging
  • Radiopharmaceutical Therapy Monitoring

Background:

  • 18F-NaF PET is a promising imaging biomarker for evaluating bone metastases response to therapy.
  • Assessing treatment response requires reliable and repeatable imaging metrics.

Purpose of the Study:

  • To evaluate the test-retest repeatability of 18F-NaF PET-derived SUV imaging metrics in individual bone lesions.
  • To compare repeatability at both lesion-level and patient-level analyses.

Main Methods:

  • 35 patients with castration-resistant prostate cancer and bone metastases underwent two 18F-NaF PET/CT scans.
  • 411 bone lesions (>1.5 cm3) were segmented and analyzed for SUVmax, SUVmean, and SUVtotal.
  • Repeatability was assessed using Bland-Altman analysis, ICC, and coefficient of variation across three imaging sites.

Main Results:

  • Lesion-level coefficients of variation ranged from 6.6% (SUVmean) to 25.5% (SUVtotal).
  • Patient-level coefficients of variation were slightly smaller, ranging from 5.3% (SUVmean) to 18.5% (SUVtotal).
  • Intraclass correlation coefficients (ICC) were excellent (>0.95) for all SUV metrics; however, significant differences in variance and mean measurements were observed between imaging sites.

Conclusions:

  • 18F-NaF PET/CT demonstrates similar repeatability for SUV metrics at both lesion and patient levels.
  • Significant site-specific differences in measurements were noted, requiring consideration for multicenter studies.
  • These findings support the establishment of 18F-NaF PET criteria for clinically quantifying bone lesion treatment response.
