Background:

  • Myelodysplastic syndromes (MDS) are a group of clonal hematopoietic stem cell disorders.
  • Transformation to acute leukemia is a known complication of MDS, but lymphoblastic transformation is exceptionally rare.
  • Refractory anemia is a subtype of MDS often managed with supportive care.

Purpose of the Study:

  • To investigate the lineage of blast cells in a patient with refractory anemia experiencing acute transformation.
  • To contribute to the understanding of rare lymphoblastic transformations in the context of myelodysplastic syndromes.
  • To explore the clonal origin of myelodysplastic syndromes.

Main Methods:

  • Morphological and cytochemical studies to determine blast cell lineage.
  • Immunophenotypical analysis using monoclonal antibodies against various hematopoietic lineages (myeloid, monocytic, erythroid, megakaryocytic, lymphoid).
  • Analysis of immunoglobulin and T-cell receptor genes to confirm lymphoid origin.

Main Results:

  • Morphological and cytochemical studies were inconclusive in determining blast cell lineage.
  • Immunophenotyping and gene rearrangement studies definitively identified the blasts as early pre-B lymphoid in nature.
