A rare mesocaval shunt, an abnormal connection between the superior mesenteric vein and inferior vena cava, was identified in a chronic cholecystitis patient. Surgical intervention successfully reduced ammonia levels and increased portal pressure, highlighting early diagnosis and interventional radiology for hepatic encephalopathy.
