Background:

  • Coats disease is characterized by retinal telangiectasias and exudation, often associated with elevated vascular endothelial growth factor (VEGF).
  • Standard treatments aim to reduce leakage and exudation, but recurrence can occur.

Observation:

  • A patient with Coats disease received six intravitreal ranibizumab injections, followed by targeted laser photocoagulation and additional ranibizumab.
  • The treatment regimen included repeat laser photocoagulation to prevent recurrence.

Findings:

  • While initial visual acuity showed no improvement at six months, significant reduction in leakage and complete resolution of retinal detachment were observed.
  • By 12 months, submacular exudates completely absorbed, and visual acuity improved to 20/30.
  • At 20 months and six years, visual acuity reached 20/20 and 20/20 respectively, with a stable retina and no recurrence.

Implications:

  • Combination therapy of intravitreal ranibizumab and laser photocoagulation is a promising strategy for managing Coats disease.
  • VEGF inhibition may enhance the efficacy of laser treatment by reducing vascular permeability.
  • This approach offers potential for long-term visual recovery and stability in Coats disease patients.
