Background:

Extramedullary plasmacytoma is a rare plasma cell neoplasm occurring outside the bone marrow.
Primary pulmonary plasmacytoma is an exceptionally rare subtype of extramedullary plasmacytoma.
It lacks systemic features of multiple myeloma.

Observation:

A 48-year-old male presented with a middle ear tumor.
Initial investigations revealed an enlarged left lung hilum on chest X-ray.
A subsequent CT scan identified a well-circumscribed solitary pulmonary nodule.

Findings:

Pathological biopsy confirmed the diagnosis of isolated pulmonary plasmacytoma.
This represents the first reported case of primary pulmonary plasmacytoma presenting as a solitary pulmonary nodule.
No lymph node involvement was detected.

Implications:

This case expands the clinical spectrum of primary pulmonary plasmacytoma.
It underscores the need for thorough differential diagnosis in cases of solitary pulmonary nodules.
Further research into the pathogenesis and optimal management of this rare condition is warranted.
