Summary

This case report details a rare instance of isolated Aspergillus infection affecting the lacrimal gland (LG) in a young male. The fungal infection presented as orbital swelling and proptosis, unresponsive to antibiotics.

Background:

  • Fungal infections of the orbit are uncommon, with Aspergillus being a potential pathogen.
  • Dacryoadenitis, inflammation of the lacrimal gland, typically has infectious or inflammatory causes.
  • Isolated lacrimal gland involvement by Aspergillus is exceptionally rare.

Observation:

  • A 23-year-old male presented with a 4-month history of right orbital swelling, pain, and proptosis.
  • Clinical examination revealed conjunctival congestion, discharge, and a palpable lacrimal gland mass.
  • The patient did not respond to empirical antibiotic treatment.

Findings:

  • Computed tomography confirmed a lacrimal gland mass.
  • Surgical excision and biopsy revealed abundant blackish material within the lacrimal gland.
  • Histopathological examination identified Aspergillus as the causative agent.
  • Systemic and ENT evaluations excluded other sites of fungal infection.

Implications:

  • This case highlights Aspergillus as a potential cause of isolated dacryoadenitis.
  • Early diagnosis and appropriate antifungal treatment are crucial for managing such rare orbital mycoses.
  • Further research into the epidemiology and management of lacrimal gland aspergillosis is warranted.
