Background:

Undifferentiated pleomorphic sarcoma (UPS), formerly malignant fibrous histiocytoma, is a common adult soft tissue sarcoma.
UPS is rare in the maxillofacial region, presenting diagnostic challenges.
The tumor comprises undifferentiated mesenchymal cells resembling fibroblasts and histiocytes.

Observation:

A 44-year-old patient presented with a painful, rapidly growing left mandibular mass.
Imaging revealed an ill-defined, heterogenous, hypermetabolic 4 cm mass invading adjacent soft tissues.
Histopathological and immunohistochemical analysis confirmed the diagnosis of UPS.

Surgical resection via left mandibulectomy with fibular free flap reconstruction was performed.
